could not open my account

i am insurance advisor in reliance life insurance.i submitted a application of account opening through a executive of icici bank canought place branch.on the date of 1-07-08.my account number is000701616802.along with it lot of account opening application submitted by another advisors.but till date my account could not be opened,nd executive not picking my and other agents phone.so pls look it on urgent basis
