DYSFFUNCTIONAL WEB PAGE

I tried to register to Bally's web page to manage my account and cancel my member ship since there is no Bally's fitness center remain in San Diego, CA. Every time I finished filling in the form, It transferred me to another page that said an error has occurred. Other time, right after I entered my address and zip code, it jumped back to the previous page and that prevented me from completing the form.
