    For:      TO REPAIR DAMAGED PROPERTY: -405 E DECATUR ST / ELM PL/ 131 ST BROKEN ARROW OK

Dear Sir or Madam:

    I have been retained by WINDSTREAM to assist in the collection of damages my client suffered as a result of your actions. My client’s records indicate you owe $1,883.65.

    This letter will serve as a formal demand for the payment of $1,883.65 within ten (10) days from the date of this letter.  Such payment should be made payable to WINDSTREAM and sent directly to this office.  If payment is not received, it will be necessary to pursue collection of this account through legal channels.

    Should it become necessary to file suit (through local counsel) to resolve this matter, in addition to the $1,883.65 in damages, you can expect WINDSTREAM to seek reimbursement for all expenses incurred as a result of this matter.  These expenses include, but are not limited to, court costs and attorney fees where permitted, plus interest.  As you might imagine, this may add a significant amount of money to the total payment you will be required to make.  You have the opportunity to avoid this by sending full payment now.

    Was this in a subpoena, delivered by hand by a process server or law enforcement officer?  Or was it emailed to you?

    There appears to be a licensed attorney by that name.
    If you have some connection to an accident at the location described, this may be an attempt to recover damages from you.  If this involved an auto accident, you should contact your auto insurance company, since they have a responsibility to defend you or cover liability for damage to property.
    Information is a bit vague.  Anyone demanding payment for damages should be able to provide documentation on the costs to repair, including invoices or an itemization of labor and materials.
    "Windstream" might refer to Windstream Communications, a VoIP and internet provider.  Did you ever use them, maybe rent a modem?
