Fraudulent account opened in my name

Received invoice #3810526004 in the amount of $246.31 for "First Bill" due December 12, 2018.  During a visit to local Verizon store to find out what this was for, learned that someone had used my social security number to open a new account.  During this same period, learned that FedEx had delivered a Verizon package to the FedEx operation center in Gilford, NH which was signed for by someone using my name and presenting an ID with my information.  Police and social security office have informed my that I need a fraud report from Verizon in order to move forward with them.  Please advise as soon as possible.  Thank you, Mark Bussiere
