Deceptive over charging

I rented a vehicle at MIA about 10 days ago and specifically told the agent I declined all insurance coverage. When I returned the car, I found that I was charged almost $100. for some insurance. When I looked at the original agreement, I saw that I had initialed a waiver statement that meant exactly opposite of what I requested
