Misrepresentation and failure to send promised rebate

In March 2011, I purchased a new cell phone from Verizon because my old cell phone was broken. At the time I purchased the phone, I was promised a $60 rebate, in the form of a debit card that was to be mailed to me. This never came tome.
I also learned later, when I changed providers, that Verizon said I had signed up for a new 2-year contract. I never knew I was being told I had to commit myself to Verizon for another 2 years simply because I needed a new phone in order to continue using the service. This situation was misrepresented to me. It is possible that some language to this effect was contained in the receipt I signed (I had to sign it, since I was using a card to pay for the purchase). I feel that this is misrepresentation.
