I was not able to print the form online, and the gym's printer was down so they said they couldn't print it for me. I found it posted online in a public domain which was helpful.

I scanned and emailed it to Alyinn listed in the other complaints. So far she hasn't responded. So, I've sent a 2nd request to Aylinn, Orlando and 2 more people whose email addresses are:

    Good news...when I sent an email to Orlando and the other 2 addresses above, I told them I had Alyinns out of office reply as proof they were receiving my emails and I was entitled to immediate cancellation.  I got TWO conformation emails that my membership is cancelled. FINALLY!

    I have recently relocated to Denver Colorado and found that there are no LA Fitness Gyms here in this state. I moved from , Mesa Arizona 3 months ago. I wish to cancel my membership since there are none here in CO. My barcode #F18990563.     I activated my membership for two months only and thought that as soon as the two months were over my membership would be canceled automatically-NOT.  Now I am back home and still being charged through my credit card.  I called the gym and they sent me a link (which does not open at all)three times in a row so that I can fill out a cancellation request , print it and send it to LAfitness.  I called to complain a fourth time and they gave me a corporate number and an employee gave me the instructions on how to get access to the form through the website but for the life of me not one single link through google would open.  HELP! my barcode is F22330548.  I just want to cancel for God's sake!  It is not fair that I am being charged monthly and i am not even in the US anymore.  Makes you think not to have anything to do with LAfitness in the future I guess.

    I am no longer in need of the services of the gym membership as there is not one within 200 miles of my new job, so effective January 19,2017 I am requesting that you cancel my account.     I registered at the gym 2 weeks ago and I only went once and it was great, but I wanna cancel my membership with full refunds because I'm going out of the states for 3 up to 5 months and I don't want to freeze my account I called the location that I registered at and they couldn't do for me.
