Washing Machine delivery and installment and No contact from Supervisors

I purchased a washing machine on 9/1/15 and requested a delivery for 9/5. When the gentlemen came he said that he could not install the machine because I need a longer hose. They scheduled my next appointment for Labor day and I was told that someone would come in the a.m. The guy never showed up on Labor Day and I called to inquire three times he would come. Then a rep scheduled me for another appointment and when this guy showed up, he said he does not do installations. I spoke to a rep that informed me that I would receive a $100 gift card within 48 hours through my email. I never received it and called back three times and was told the same thing. I requested to speak to a supervisor and was told that a supt. would contact me. No supt. called me. This happened twice. I am requesting a supervisor contact me to explain why I have been ignored?
