Damaged bar stool, tv & audio equipment

On June 26, 2012 your rep did a repair at our house. However he replaced the receiver with one that did not record so I called Direct tv and ordered a DVD that recorded and at the same time reported that the DVD that was replaced a couple days before was not properly working in that I now had double images. I requested the same service person but was told not possible.
On June 30th a different rep. Came to our house but not only had no idea what to do to fix the problem he knocked over a custom made lucite bar stool & damaged the stool. A picture of the stool is available. It was knocked over when the rep was looking for his ID and I told him it was on his head. When he reached to get his ID he moved into the bar stool and knocked it over striking the marble kick plate of the bar resulting in a chipped stool.
This incident was immediately reported to his co. I received a call from Victor who resched. an appointment at my house today. Jessie appeared at my home and was unable to correct the problem and advised me to file a claim for both the equipment and the bat stool.
