They called my insurance company and told them that i was receiving benefits from another state. They left the number 1-866-498-3790 ext. 4129 and said thier name was Brian Mitchell. So my insurance company called me and told what had happened and gave me the number, i have called from my house phone and they blocked my number where i could not call back, so i called from my cell phone and eventually they block me from that number too.
Comments
1) Harassment of some sort. Does anyone hold a grudge against you, or is anyone harassing you in other ways?
2) Misidentification, assuming you are not actually receiving benefits in some other state.
3) Id theft.
Because of (2) and (3), you may want to look into this further. Did your insurance company indicate what the name of this company was?
When insurance companies pay benefits, whether they are medical, or even if they pay on your house policy to repair damage, they report it to some insurance databases, to make sure that there are not multiple claims filed for the same thing, an indication of possible fraud. That might lead one insurance company to contact another insurance company, if they are dealing with a claim, and it appears thru your file that more than one person may be using your identity.
Just as you can pull your credit reports to see what is on them, you can also pull reports from these insurance databases. It appears that you can also get 1 free report a year, just as you can from the big three credit reporting agencies.

The FCRA sets the standard for the CLUE report just as it does for credit reports. CLUE reports fall into the FCRA definition of "consumer report," that is, a report bearing on your "credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living." ChoicePoint, the major provider of property loss reports, is a "consumer reporting agency" under the FCRA. That's because ChoicePoint "regularly engages in . the practice of assembling or evaluating consumer credit information or other information on consumers for the purpose of furnishing reports to third parties, and . uses . interstate commerce for the purpose of preparing or furnishing consumer reports." (15 USC §1681a(f))
Under recent amendments to the FCRA, companies that compile insurance claims reports have been further defined as "nationwide specialty consumer reporting agencies." Does the FCRA give me any rights?
Yes. When it comes to your CLUE report, you have the same rights as you have for your credit report. In a nutshell, you have the right to access your report and to dispute inaccurate or incomplete information. You are also entitled to notice about an adverse decision based on information in the report. For more on your rights, see Part 5 of this guide. ..."
You didn't say what type of "benefits" these were. There is also some database used to track medical benefits, which has been used by insurers to deny "pre-existing conditions".
Your insurance company should be able to help you get copies of reports from any insurance databases, or at least point you to who to contact.
The third place to look is for indications of more general id theft, such as fraudulent accounts, use of SSN, etc. Get a copy of your Social Security statement from the Social Security Administration, to see if someone is working under your SSN. Get copies of your credit reports, to see if there are accounts on them that are not yours, or name variations, SSNs, DOBs, and addresses that are not yours.
