charging on closed accnt

had a membership with A.V. Fitness, Lancaster CA.  no auto pay, always a statement and paid by phone. 8/14 i Physically walked in, signed form and cancelled, they stopped billing.  while a member,  I did change banks,  billing and payments remained the same. Currently (2yrs later) A.V. Fitness and my bank have been taking automatic payments without my consent, Reported to both but still don't stop.
