Early termination fee

Last fall I called Dish Network to place my service on vacation for the winter while in Florida. I specifically asked the operator if the monthly charge would increase because of this? I was told that it would not.
This Spring I returned and called them to have the service turned back on. I was told that it would but there would be a substantial increase in the monthly fee since my putting it on vacation negated the introductory rate from the fall before when I first started with DISH. I balked at this and told them to cancel my service. They then informed me of the early termination fee that I would be charged ($200.00). I asked them for a copy of my signed contract since I never signed a contract. I was placed on hold for a lengthy period of time. The operator came back finally and said she just talked to her supervisor and that they would waive the early termination fee. She then reduced the monthly fee to the original fee and told me that she would give me this special rate for the next 5 months. I accepted.
I called them again today to cancel service. They again told me that I would be charged an early termination fee of $200.00. I referenced my call to them this spring but the operator could find no record of the waiver I was offered this spring. I asked what date they have my contract expiring and was told is was 16 Apr 16, exactly one year after I called them this spring. No one told me my contract would be extended during that phone call.
I demanded a copy of the signed contract and was told they would e-mail or mail it to me in the next couple of weeks and that if I did not receive it I should call them and start the process all over. I asked to speak with a supervisor but was told there were none available. Meanwhile, I still have DISH and am still paying the same monthly fee.
