I cancelled it right after the form I needed was not on your site

The moment I signed up, and saw that you did not carry the form I needed I immediately cancelled my service. Now, it is May-2017,  and I got my April CC statement and i am STILL being charged for something that I cancelled on the same day I sign-up. Even the phone number on my statement say the phone number is 'Out-of-Service' (866) 816-3618.
