no phone number to call as stated on statement and no free git with order

I ordered a quiting book from homes and garden called complete guide to quilting. A free gift was to be included not in pkg. Also it states on back of invoice"simply contact us at number or address on front of your statement"no number or address just a return package address and That leaves me no way to cancel or find product and question bill I need help. They have my charge card number that was requested to get my free gift and a bill
