I have been trying to get a death certific for my father (HUGH T JONES) for abouth a month with no success.The i have talked to a rep of your company several times and first she told me that she would send me my email the form to fill out, never recieved it
then i called again and taked it could be sent my mail was told twice that it would me sent by mail out that bay, HEVER RECIEVE IT. YOU HAVE MY MONEY WHY DON'T I GET THE FORM????????????
