credits due not applied

I have been arguing with Verizon wireless customer service since February regarding charges to my account.  
We had phones turned off but they continued to charge us, then once i went on line and turned them off, they never have issued credits.
We have upgraded a phone which was supposed to be free, instead they added a new line and have been charging us for 2 lines on the same phone.  They refuse to remove it because they say both numbers are under 2 year contract.  They sent me a copy of the receipt they say we signed and it is clearly not our signature, it was a store employee.
2 months age we started usingan account rep that will not return our emails or calls.  how do we get satisfaction and get them to stop sending us past due balance when we don't owe it and issue the credits they promised.  They say we have a past due balance over $550.  I would switch providers but we have over 20 phones on our business account.
