They did the refi without knowing my correct name

After approved by Bank of America for a loan, we did two closings so far still without any result. First one is the mistake spelling of my name on every pages. A few days later, we faced  two versions of names on the closing documents. I can't believe it happened to a big bank with a certain reputation. We are still waiting for the closing...
