Unable to deactivate

Every day since a month this facility has be deducting 5 rs from my account. Please deactivate this as i do not require this anymore. No facility seems to be present, so as to unsubscribe this over the internet

I have called in customer care 4 times (22,24,25,27
april) and till date I had lost rs daily from my
balance.
Every time the customer care executive says that it has been deactivate,but I am not satisfy with it.
This is still running for my phone and rs 5
deducts every day.
Please help me as soon as possible my no.
