after 4 month of cancellation they still charging my debit card

I cancelled my service on April 24, 2013, I paid my last balance and they sent me a box to return the equipment, I did it immediately, a few weeks later I received an email charging me $18 for the UPS returnig  service. I called and a very nice lady told me she is removing these charges and that will be the end of the story.
A month later same thing, an email announcing my debit card will be charge for the amount of $18, I called again, the guy told me that for some reason the system didn't take the previuos adjustments, but he is doing it again and it should be all.
Ok...today I received a notification that my debit cardwas charged $38.75 from Dish network....the $18 plus a past due amount.
I could not believe and called again, spoke with a supervisor who said (again) everything is OK now, but in the midtime, they have my money, I even have to call again tomorrow to request my refund and wait a couple of days (maybe more) to have my money back in my account.
This is ridiculous!!!
