On 3/8/2016, I placed an order for my husbands birth certificate. On the same day I received anothe copy that I was to complete a state form with a copyof my photo. I do nt have a photo to send you nor the means to get one. On 3/8/2016 I received a processing fee for the birth certificate that I DID NOT RECEIVE !!   I had NO intention of becoming a member Archives (order #531190252. This is Not my order # for my original request. Nor do I appreciate being charged monthly fees for something you DID NOT Privide an d Placing me somewhere that I did not request. Please CANCEL  these two order #'s immediately. I have not been able to contact you prior to this date due to an auto accident, that totaled my car, sent me to the hospital's trauma center, etc. Please respond to < marsclair@optimum.net >  ASAP Thank YOU
