Re: 1. Request for complete membership cancellation.
2. Request for cancellation of automatic debit.
3. Request for refund.
EFT Department or Refund / Cancellation Department,
This letter is in regards to my membership xxxxxxxxxxxxx. From my complete knowledge, I signed up for a 1 year membership to Bally Total Fitness by phone on June 13th, 2009. Without my knowledge or consent my credit card was billed on June 15th, 2010 for another full year membership at the cost of $116.37. In June of 2009 I signed up for a 1 year membership without the consent of an automatic renewal. I would like to cancel my membership fully, stop the automatic electronic funds transfer, and be refunded the entire amount of $116.37 that I was wrongfully debited. I have already spoken to a customer service representative by the name of Rebecca, ID# 1687, and she has instructed me to compose this letter and mail it to the above address in the attention of the Refund / Cancellation Department. She then said a full refund and confirmation letter will be issued back to me by mail. This letter is my official written request to have my membership cancelled, the cancellation of automatic debit and request for a full refund. I would also like the written confirmation from Bally Total Fitness of the cancellation and refund as stated by Rebecca, ID# 1687 to my home address located at the top of this letter.
Thank you in advance,
I have my fingers crossed. Bally's was the worst experience. Dirty gym's, bad staff and scam artist company that take your money without asking.

Yes, I used certified mail with the letter adding all information I could. I used Mike's address (corrected).

I've got a friend who lives in a location with no Bally's within a 25 miles radio, with whom a lived for quite some time (currently I don't though). I've still got the old address in my driver's license card; would a photocopy of it work? I don't have my Bally's card though, would the number be enough?

I am over it. I am losing out on training sessions, but at this point, I don't care.
